Referrals
Referrals can be made by service users, family members, health and social work staff, voluntary agencies and support groups.
Referrals are made to the project administrator at WIAMH. Once received, referrals are anonymised and considered by the project committee. Referrals may be referred back for amendment, but once details have been agreed, funding can be made available for the support to be put in place.

Using the Referral Form On-Screen
The form is in PDF format. Most PDF reading software (e.g. Acrobat Reader) will allow you to fill the form in on-screen. (The current version has been updated so you should be able to save it on your own computer and send it by email.)
Please note that we are unable to accept final submissions of the form unsigned by the referrer (contact us if you would be interested in using a digital signature).
